An INTRADISTRICT Attendance Agreement is ONLY for those families who already reside in the Eureka Union School District boundaries. These agreements are required to be renewed annually. Please complete one form per child.

Moved out of EUSD - Continued Enrollment
If you have moved out of the Eureka Union School District boundaries, while your child attends one of our schools and you wish your child to continue at that school, you will need to fill out an Interdistrict Transfer request from your new District of Residence. Your child may continue attending their current school during this process.
Procedure
- Contact your new School District of Residence to obtain and complete their Interdistrict Transfer Request form. Under Reason for Request, please select Continued Enrollment.
- You will be required to provide proof of attendance which is best obtained from the EUSD school that your child currently attends.
- Their designee will approve and release the transfer and will email the form to the Eureka Union School District.
- Once received by EUSD, the form is entered onto a spreadsheet that is shared with the school sites for their review.
Renewal for the Next School Year
The Eureka Union School District is always eager to encourage new students from neighboring districts interested in our district. We welcome you to explore our website and contact the district office or school sites for further questions or information.
Interdistrict Transfer Requests from students outside of our District to come to our schools are accepted on a space available basis. The student must maintain good academic standing, good attendance and proper behavior as determined by the principal of the school of assignment.

Procedure
- Go to the website for your School District of Residence to obtain an Interdistrict Transfer Request and fill out their form.
- Their designee will sign and date the release and they will email the form to the Eureka Union School District.
- Once received by our District, it is entered onto a spreadsheet that is shared with the school sites for their review.

Interdistrict Transfer Request Application
We ask that you attach documentation as proof for your reason for the transfer(s). Examples of documentation are: attendance records from your student's school, a letter from your employer or childcare provider, etc. NOTE: Failure to submit all of the required documents will result in the denial of your interdistrict transfer.
