Registration Documents
To finalize the registration, please provide the items listed below. The preferred method is uploading these documents during the online registration process. If you are unable to upload them, you can also deliver them in person to your school of residence or requested school.
1. Student's Original Birth Certificate
2. Proof of Residency
The parent or legal guardian must provide the following documents in their name for residence verification: (If the utility bill is not in the name of the parent or legal guardian, please contact the school site directly for an affidavit.)

- Utility bill,
- Garbage bill,
- Home telephone bill (cell phone bills are not acceptable), or
- A letter from Social Services verifying residency (must be dated within the past 30 days)

If the family has recently moved and does not have a utility bill yet, we will accept one of the following:

- Purchase Agreement, Lease Agreement or Rental Agreement
- Lease and rental agreements must include the contract start and end dates and the signature page.
AND one of the following utility-related documents within 30 days of the move in date:

3. Current Immunization Records
Children will only be permitted to attend school if a documented, up-to-date immunization record of required immunizations is submitted to the school. Please contact your healthcare provider if your child’s immunizations are not current. Below are links to informational guides regarding immunization requirements:
4. New Student Health Document Requirements

- EUSD Health Registration form - All families of new students must complete this form, which helps the school provide tailored support for any health-related needs.
- Report of Health Examination for School Entry (CHDP) form - The CDHP form must be submitted before your student’s first day of 1st grade. Once completed, the form is valid for 18 months, allowing it to be completed before your student starts kindergarten. This form must be filled out by an authorized medical provider, then signed and stamped.
- Oral Health Assessment form - This form must be completed and signed or stamped by a dentist prior to May 31st of your student's first year in a California public school.

If your student requires medication at school, please review the grade level guidelines linked below. Submit the grade level authorization form to your healthcare provider for completion.
